"if any of the .. criteria IS met" is CORRECT grammar, because is relates to any, not to criteria. "any of the following criteria" = "one single criterion out of the following criteria", now you should see, that "one criterion is..." is correct. Another way is just to understand the meaning of the sentence, one criterion is to be met by the player, not more than one.
