You can get the code from here, it'll list it for you. Just fill in the values (without changing the labels) - it should look something like this...

I will find additional references and add them. Please note this athlete played in the same league as many other athletes featured in Wikipedia, even though not a top league. However, if you feel you can find enough reliable sources which talk directly about Peter Chamis, then I will be prepared to WP:Userfy (that is copy the contents into a space on your userpage) Peter Chamis (soccer) for you and assist you to build it up so that it can be moved back into mainspace and not be deleted again.
