Pegcetacoplan, sold under the brand name Empaveli among others, is a medication used to treat paroxysmal nocturnal hemoglobinuria (PNH).[2][4] Specifically it is used in adults who continue to have low red blood cells despite taking a medicine called a C5 inhibitor.[4] It is given by gradual injection under the skin.[4]

Common side effects include injection-site reactions, infections, diarrhea, abdominal pain, respiratory tract infection, viral infection, tiredness, and fever.[2] Safety in pregnancy is unclear.[2] Use requires people being vaccinated.[4] It works by binding to and inhibiting complement protein C3.[4]

Pegcetacoplan was approved for medical use in the United States and Europe in 2021.[2][4] In the United Kingdom it costs the NHS about £24,800 for 4 weeks.[5] In the United States this amount costs about 37,000 USD.[6]
