Plot[edit]

Death row inmate Ray Kaspo is killed by poison in his last meal, 45 minutes before his scheduled execution. Adrian Monk is called upon to investigate the case by Captain Stottlemeyer, despite the detective's fear of prisons. Adrian is unable to bring himself to investigate the case until he receives a phone call from Dale "The Whale" Biederbeck requesting a meeting. Dale, the prime suspect in the case, tells Adrian that he is innocent and that he will give Adrian information on Trudy Monk's murder in exchange for solving Kaspo's death. Although Adrian's nurse, Sharona Fleming, is skeptical, Adrian accepts Dale's offer.

While investigating the prison's kitchen, Adrian and the prison staff discover a cook dead in a freezer with money in his pocket, indicating that he poisoned Kaspo and was paid to do so. Adrian receives a note from the prison librarian, Sylvia Fairbourn, asking to meet. In the prison library, Adrian and Sharona see a newspaper article stating that a billionaire, Lambert Lawson, has died from kidney failure. Sylvia tells Adrian that Kaspo was in the library the week prior to his death, discussing an unspecified incident in Calgary with another inmate named Darnell "Spyder" Rudner. Adrian resolves to go undercover as an inmate to gain information from Spyder.

After winning Spyder's favor by retrieving a watch from a group of neo-Nazis, Adrian asks for information about the Calgary incident, but Spyder reveals that he has never been to Calgary and has never met Kaspo. Meanwhile, Sharona learns that the amount of poison in Kaspo's body was far beyond what was needed to kill him, in fact completely destroying all of his organs; and that Kaspo was an organ donor with an extremely rare blood type.

Adrian returns to the library to confront Sylvia and sees her watching a television interview with author J.T. Demornay, who was being sued by Lambert Lawson for seventy million dollars at the time of the billionaire's death. Adrian is told by a prison officer that the warden wants to meet him in the prison's recreation room, but the meeting is an ambush planned by Sylvia and the neo-Nazis, who pursue Monk through the prison. Meanwhile, Sharona receives a call from Lieutenant Disher informing her that Lawson had the exact same blood type as Kaspo. The prison's warden enters the office Sharona is in, asking for Monk, alerting her and the prison guards that he is in danger.

Sharona summarizes the case to the warden: After Kaspo's execution, his kidney was intended to be donated to Lawson as they had the same rare blood type. Sylvia, Demornay's mother, sought to destroy Kaspo's kidneys to indirectly kill Lawson and get the lawsuit against her son thrown out, bribing the cook to spike Kaspo's last meal with enough toxins to do so. Adrian is cornered in a laundry room by the neo-Nazis, but is saved by Spyder, who beats them up as the guards arrive.

Having completed the favor for Dale, Adrian asks for the information he has. Dale tells Adrian that the bomb that killed Trudy was intended for her, not Adrian as the detective had previously believed; that a man named Warrick Tennyson in New York was involved in Trudy's death; and that Tennyson is in New York, prompting Adrian, Sharona, Stottlemeyer, and Disher to fly there.
